The hazy image (see attached) was uniformly hazy -- much like lens flare -- with soft halos around highlights in moving videos. I couldn't discern anything on the lens cover. But after removing it and examining it closely, it appeared to have a very consistent and subtle film of something on the outer surface.

Excellent advice. After about 5-10 minutes of first inserting a sheet of paper between the lens cover "louvers" I got brave and used a dental pick as well. The paper alone would not free up the louvers, but using the dental pick and GENTLY prying I was able for free-up the operation of the lense louvers.
